About the information we hold<\/strong><\/p>\n

To help meet your health care needs we may hold and use records, including your name and address details, your<\/p>\n

medication record and other health care information that you or your treating doctor or other health practitioner may<\/p>\n

need to provide to us. This personal information will not be disclosed to overseas recipients and will be used only for the<\/p>\n

purposes of meeting your health care needs, unless you consent to its use for other purposes, or the law permits its use<\/p>\n

for other purposes.<\/p>\n

Much of the information we collect must be collected because the law requires it. For example, details of your<\/p>\n

prescriptions and, in some jurisdictions, the supply of medicines containing pseudoephedrine, are required to be<\/p>\n

collected under relevant legislation.<\/p>\n

For some services we will not need to collect any personal information. You need to be aware that if you do not<\/p>\n

provide us with certain information about yourself, in some circumstances we may be unable to provide a service that<\/p>\n

you require.<\/p>\n

Direct marketing<\/strong><\/p>\n

If we hold information about you, for example, as part of a loyalty program, this information may be used for marketing<\/p>\n

purposes. If you do not wish to receive marketing communications from our pharmacy you can tell us and we will take<\/p>\n

steps to have your name removed from our pharmacy\u2019s loyalty program marketing list.<\/p>\n

Our disclosure and information practices<\/strong><\/p>\n

In accordance with our Privacy Policy and relevant policies such as our Confidentiality Policy* and our Computer Access<\/p>\n

and Security Policy*, personal information we hold is stored in a secure manner and is not disclosed to any other person<\/p>\n

or organisation without your consent. However, we may need to provide your information to other health service<\/p>\n

providers if it is necessary to help us to provide services to you.<\/p>\n

Special pharmaceutical services and activities<\/strong><\/p>\n

Some activities such as Medication Management services or research activities will have privacy practices and consent<\/p>\n

forms specific to the activity. If our pharmacy is part of the team involved in that activity, we are required to meet our<\/p>\n

privacy obligations for that activity whether they arise under the Act or otherwise.<\/p>\n

 <\/p>\n

How you can gain access to information<\/strong><\/p>\n

You can request access to personal information we may hold about you. If you make such a request, we will ask you to<\/p>\n

complete a form for this purpose. Most of the information we hold comes from the prescriptions presented to us.<\/p>\n

You can also request that corrections be made to your information we may hold about you. There is no charge for<\/p>\n

lodging a request to access your information. However, we may make a small charge for our time involved in providing<\/p>\n

this information and for associated costs such as photocopying.<\/p>\n

Complaints<\/strong><\/p>\n

If you have a concern please raise it with us first as we may be able to resolve the issue. You can speak to the pharmacist<\/p>\n

in charge or contact us at the above address. If you are still concerned, you may lodge a formal complaint with the<\/p>\n
